Ryan White HIV/AIDS Program

Funds reach recipients through several distinct routes by Part. Part A is awarded by statutory formula plus competitive supplemental and Minority AIDS Initiative (MAI) awards directly to the chief elected official (mayor/county executive) of Eligible Metropolitan Areas (EMAs) and Transitional Grant Areas (TGAs), who then subaward to local HIV care providers. Part B is awarded by statutory formula plus competitive supplemental and MAI awards directly to the 50 states, DC, Puerto Rico, the U.S. Virgin Islands, Guam, and the Pacific jurisdictions, which administer services and the AIDS Drug Assistance Program (ADAP) and subaward to local providers. Parts C, D, and F are awarded competitively through HRSA Notices of Funding Opportunity directly to clinics, community-based organizations, and training centers.

Authority

Ryan White HIV/AIDS Program is authorized by Public Health Service Act, Title XXVI (originally the Ryan White Comprehensive AIDS Resources Emergency (CARE) Act of 1990), Sections 2601 et seq. (PHS Act Title XXVI), administered by HHS / HRSA, as a mixed program. Statute.

Part A and Part B base/formula awards are calculated under a statutory formula defined in the PHS Act using the number of living HIV/AIDS cases reported to and confirmed by the CDC. For Part A, over 66% of available funds are awarded by formula based on estimated living HIV (non-AIDS) and AIDS cases in the EMA/TGA, with the remainder (less hold-harmless amounts) awarded as discretionary supplemental grants based on demonstrated additional need through separate competition. Part B base grants depend on the number of people living with HIV in the jurisdiction; the Minority AIDS Initiative (MAI) component is formula-distributed to areas with high HIV/AIDS rates. Part A recipients must spend at least 75% on core medical services and no more than 25% on support services. Effective FY 2026, HRSA is transitioning the Part A and B formulas from counting cases by address at HIV diagnosis to counting living HIV/AIDS cases by a person's MOST RECENT address, phased in over five years (FY 2026 through FY 2030); this change was set out in a Federal Register Notice published November 10, 2025 (90 FR 50747).
